Questions Before Selecting an ASSISTED LIVING
or MEMORY CARE Facility

Identifying significant changes may help to make an informed decision about considering senior living options, such as assisted living or memory care.  The following are questions you may consider to help determine whether you or your loved one can benefit from an assisted living or memory care facility.

  1. Is there difficulty performing familiar tasks?
  2. Is there depression or disinterest in previously enjoyed activities?
  3. Have you noticed unusual forgetfulness, such as forgetting the name of a loved one?
  4. Are medications being taken correctly and on a timely basis?
  5. Have there been changes in personal hygiene?
  6. Are there changes in personality, mood or behavior?
  7. Are there unusual piles of unread mail or magazines/newspapers?  Finding spoiled, outdated foods in the refrigerator?
  8. Have there been 2 or more recent falls and/or visits to the emergency room?
  9. Has there been rapid weight loss or gain?
  10. Has there been confusion /disorientation in familiar places, or getting lost?
